Breach of contract// Claim delay!!!

I called the FNB Device Insurance division on 2013/ 06/ 18 to make a claim for my lost/ cellphone.
I was advised of a 48 hours to 5 days turnaround time but I have been waiting since. I was informed that an FNB Insurance division agent will be in contact and I was to have my ITC Ref no and case number ready. Instead of getting a call taking my details and processing my claim as per standard procedure and timeline, I received a copy of the insurance contract that I was meant to receive within 30 days of insurance inception.

The insurance policy inception date is 2013/ 03/ 01 but the documents that were meant to be sent to me within 30 days of inception were only sent to me on 2013/06/21, negligence on the Device Insurance divisions' side. Further more, on 2013/06/26 R122, 20 was debited from my account, R97, 76 categorized as pro-rata and R24, 44 as normal premium without my permission. The debit date as per agreement is 26th of every month, for an amount of R24, 44 not once did the FNB Device Insurance division debit or attempt to debit my account. To me this act seemed like a blatant cover up of the initial breach of contract committed by FNB Device Insurance Division.
