ZAR 000028668_MB - repeated calls for payment

I refer to account ZAR 000028668_MB. I am continuously receiving requests for payment on this account! The debit order has been deducted from my bank account every month up until 28.06.2013. The amount is for R159.00. I can no longer put up with this as I am told that I have made arrangements to pay this account which I never have done!!

I would like this contract to be cancelled immediately as I can not put up with the constant harassment. I never receive statements to show why I owe money, I want written statements emailed to me before I even consider making any payments. I have managed, after 6 months to finally sort out my other contract and wish to sort this out immediately so that I never have to deal with you people again.
