Still debited over a year after cancellation

I surrendered this policy over a year ago and I am still being debited
through stop oder - per my payslip. They keep asking me to submit proof of account which makes no sense. I do not understand why i should resubmit proof of bank account again when I did this a long time ago.
Please process my refund and cancell the ongoing payments. Event spoke to Ms Wezi Khumalo on 4 July 2013 who was not even apologetic towards my dissatisfaction with their service.
