NEOTEL my identity for a contract

A buddy and I are starting up a company. The company is fully registered and everything, but we are not operational as of yet. My buddy then decided to go get a telephone contract with Neotel. He applied in the company's name. He then sent his personal bank statements and ID doc to accompany the application. Note that I am not an applicant (neither main nor co-applicant). My name was attached to the application as a contact number. All of a sudden this Tebogo Lesiba went and accessed my credit record and came back saying the application is being finalized in my name. How come??? How can someone do this? Where did he find my identity number since it has never been part of the application? How can he do a credit check on me without my consent? This cannot be legal. This is identity. Tebogo is illegally pushing a sales. And if this application is not cancelled today the 30th of april, comes the 2nd of May I will take action.
Kindly have my names, ID numbers, and telephone numbers completely removed from your databases.
Please coorect this with immediate effect. And please teach Tebogo Lesiba how to conduct proper sales.
