Misleading Service and bad attitude

I opened a contract at Maponya Mall on the 20 /09/2012. I confirmed that I instructed the consultant to insure my cell phones.
On the 28 January 2013 my Blackberry 9320 Curve was, I followed the correct procedures and process as instructed by MTN Maponya staff. I was also issued with insurance claim forms to fill and fax to 24hr Mobility (086 6580 995).
Later I received a call from MTN maponya informing me that my phone was not insured. The owner (Lerato Makopo) of MTN Maponya also called me to also inform me that my phone was not insured, she asked for my contract by then I had misplaced it, and I asked her to give me a copy from their files they refused and she was very arrogant and unethical. Luckily I found my contract.
I was surprise; I strongly feel that MTN did not treat me with right to fair and honest dealing. There was a false, misleading or deceptive representation.
