Continued charges despite migrated contract

In September 2012 I informed Jabulile Ntshalintshali of MTN that I would like to migrate my contract to pay as you go (my contract was due to expire) and was assured that it would be migrated and that all of the paperwork had been received and was given the following reference number - 5750755. When the money continued to be deducted from my account I sent a follow-up email and Jodi Spogter responded requesting my contract to prepaid migration form again. I sent the form to her (REQ number 1529345) but didn't ever receive a response. The money continued to be taken out of my account every month. I then contacted an MTN representative via the chat function on the website and the representative assured me that I would be sent an email the next day, she then left the chat before giving me a reference number.
Mtn seem to be happy to ignore me and continue to illegally take my money.
