Mission to BlackList Phone

My phone's been more than a week ago - I urgently need to blacklist the phone, to receive and ITC number, to open a police case, to get a case number to be able to claim from insurance.
I've explained this to various people at Nashua Mobile and yet that don't seem to be able to assist me.

I did buy the phone independently and they don't have record of the phone on their system - I however sent them a copy of the box with serial number but they won't accept it as proof of ownership (both the police and insurance company said it's enough proof)

My issue is that I'm being sent from pillar to post, everyone is giving me a different story and no-one seems to want to help me.
