Failing to RICA my simcard

During November I took out my fourth contract with MTN - for an Ipad. I have three existing up to date Rica'd sim cards with MTN and all 4 sim cards are under the same contract. I produced all the documentation required of me to enter into the contract as well as to Rica the sim card. I have not been using the sim card due to "connectivity" issues and I eventually phoned and found out that this simcard is not Rica'd. I now have to get documentation together again to go and Rica it at a store (after I had already supplied that store with all the info). Strange thing is - I use my MTN invoices as my "proof of residence" - so now I am really stumped as to what they want from me to get this sim card Rica'd. They have been paid every single month for my use of the simcard even though I haven't been able to use it and they refuse to assist me over the telephone (despite having answered all the security questions) - I HAVE to go into the store according to them.
