Handset Claim Rejected but no valid reasons

ON the 19th of December 2013, I sent a claim for my Handset Samsung Note II N7100. On the 8th of January 2014 after phoning every other day, I received a rejection email notification from Angelique du Plessis citing that there was no record that I used the insured SIM card on the handset. She attached an unsigned copy of the insurance forms documents as supporting reasons for the rejection. I then replied and asked her to send me a copy of the signed insurance forms with my signature, including proof that the '0810257401' number was never used on the handset, plus a record of the conversation I had with an agent when taking on the insurance. I also phoned the call centre insurance asking for the above mentioned but I have not received the information to this day. The reason I am asking for the information is because I never signed any insurance forms when taking the insurance, I was phoned by an agent and we agreed telephonically and that was it. I have proof that I paid for handset insurance and SIM insurance. Furthermore I pay separate amounts for handset insurance and sim insurance. In this case I was claiming for Handset since I paid Insurance for it.
