Still waiting for feedback on my Insurance

I have been on hold with your insurance department for 30min, each day since Tuesday.
There does not seem to be anyone there answering the phones... all I am doing is wasting airtime!

I need assistance with my screen damaged phone.

I filled out all the forms at MTN Morningside on Monday! (The forms are with me) I am STILL waiting for someone to come collect the phone and give me a loan phone.

When I called the call center it says you still have not received my form! Apparently it should take between 24-48 hours, that was Monday... today is Friday!

Please assist ASAP.
I pay insurance EVERY month, no compliant, when I need your help I seem to have to jump through hoops, I have even asked for assistance on FaceBook and still no reply or any sort of assistance!!!

Please can someone call me regarding this issue!
