Callsure cellphone insurance claim

On the 26th of June 2013 I took out a cellphone contract with Mr Daniel Marope at the MTN store in randburg square and he offered me callsure cellphone insurance for R124 and i took it and signed the insurance form. Six months later the phone got and i went to the store to get the insurance numbers and when I called callsure they do not have my details and when I went back to Daniel Marope I requested to see where he forwarded my insurance application form and there was no proof on his P.c and other forms from the 26th and 27th of June were sent but mine was not sent and now Daniel Marope and the store manager are giving me the run around and they do not want to give me their seniors numbers. I have gone to the store several times and still no help...
