Overcharged. Telkom refuse to apply credits

I have signed up with Telkoms ADSL Special of R399 per month. I have signed for a 384kb/second line. Telkom, without consulting with me upgraded me to 2mb/ s line and charged me for the new service. I called them to correct it and asked for a credit in Nov 2012. They, instead of correcting it, upgraded my line again to a 3mb/s line and overcharged me even more in Dec 2012. I called again with much frustration and spoke to Kenelwe at the Bloemfontein branch with her supervisor Bianca. They promised me a credit to come through in Jan 2013. I received my bill in Jan 2013 and guess what. No credit and overcharged. The service at Telkom is non existent, very frustrating and an absolute shame. So what should I do. No option but to cancel my contract and will definately cancel my business contract if this is notbtaken care of in the next 72 hours.
