No feedback from regional manager - no insurance!

Mr Engelbrecht bought a Samsung 3 phone from the Keywest branch (MTN) in April. Mr Engelbrecht clearly asked Tlego (salesman) to add insurance on the phone, but due to a computer problem on the day, he could not give him the necessary documents. He however promised to email it to Mr. Engelbrecht the next day. He never did! On all the MTN statements Mr Engelbrecht noticed a Promo Service fee and assumed that it included the insurance.
Three weeks ago the phone was in Potchefstroom (the case number is available from Mr Engelbrecht). He immediately contacted MTN to block and blacklist the phone. On the Monday morning he phoned the Keywest branch and spoke to the specific salesman. He promised Mr Engelbrecht that he would look into the matter and get back to him. He never did!
Mr Engelbrecht then drove to Keywest from Magaliesburg. The salesman contacted the MTN insurance line with no success. He promised to keep trying and get back to him, but never did.
Mr Engelbrecht then contacted Samantha, branch manager. She investigated and said that no insurance was activated. She referred me to the regional manager who does not reply to my any of my correspondence.
