I did not migrate

I sent a letter to cancel my contract that expires in July 2013.
However, I received a statement dated 01 April 2013 stating that I have migrated Telkom Mobile and that I received a Samsung Pocket phone, I know nothing about this and I did not receive a phone.

I am not interested in upgrading or migrating, all I want is for the contract to duly come to an end this month.
I will not be liable for any costs in regard to this migration or the Samsung phone as I did not receive same.

Please provide written proof of the termination of this contract.
