Billed for returned USB Modem

On the 15/04/2013 I ordered a USB modem on 2GB/month contract from MTN Direct. After the device was received (18/04/13) it failed to operate. I then contacted MTN Direct to have the device returned (23/04/13) and a refund passed as I was still within the "cooling off period".

To my horror I find that I was still billed for this device and when I called MTN SP they told me the SIM card is still active (16/05/13) and they have no record of the device being returned and request to cancel and refund.

I was in constant contact with MTN SP and MTN Direct (Glasson Markham) to have this rectified but the feedback was terrible and each department keep referring me to the other. In the end MTN Direct put in a request to cancel the contract but now MTN SP say that no request for refund was logged.

How can a contract be cancelled and refund not be passed? The level of service I have received is pathetic, as a MTN SP customer of over 7 years this is the last straw.
