What part of cancell dont you get?

Contract with this company expired October 2012. I requested a migration to a lower package. After being charged a number of fees, including a fee for a new instrument which I never received I requested the contract (expired) be cancelled and the credit due to me, be refunded. This was January 2013. This was never done and since then I have been trying to get them to cancel the account but they keep invoicing me for a service fee offsetting it against my credit due. What does one have to do to get these absolute incompetent people to act??? Is the next step Icasa? Consumer protectorate? Or just as much bad press as possible?
