Contract gone missing and cancelled on the system

On the 28th of April I walked in to the mtn store at Centurion mall. Kagiso helped me fill out the application form for a new contract for the Sumsang S4. I was told that I will get feedback within 4 days. No communication was made apart from the one SMS informing me that the contract is being processed. On the 5 of May 2013 I went back to the mtn store and found another guy there, when I enquired about the progress of my contract application, I was informed that the system took too long to process therefore it was declined. When he tried to re load the application, the paper work was no where to be found. I was told that Kagiso will give me a call and he hasn't.

Please help, cause I'm very loyal to mtn and this service isn't good for me to stay loyal to the brand.
