Cancellation unprocessed for 6 months, many calls

I cancelled my membership at Fourways Planet Fitness on 1 August 2013 and forwarded a confirmation email. This was not processed. I followed up with a call in October during which they said they forgot to change my details and it will be reflected in November. On 9 January 2014 I was refered to their national call centre. I have spent more than 2 hours on calls and in the process I have sent 2 more mails to get the membership cancelled and the R2000 they have taken off my bank account back. Today they refused for me to speak to a call center supervisor to escalate the issue to.
