Second request for Cancelation

This is a second request.
My membership was not cancelled as requested during December and payment was taken again.
Im not available via telephone so please email me if you have queries.
I made contact with the gym and was informed that I must cancel with Viatility. Vitality told me I must cancel directly with the Gym as I did not join pre 2010.

Please ensure you cancel my membership with immediate effect.
