Cancellation of account

I am a body lab member in Durban cnr monty naiker & joe slovo. I have been a member for almost a year now I joint in September 2012. I was joint in by a lady named Nondumiso, of which I didn't fill the whole contract myself she did part of it as I gave my details over the phone because I wasn't sure whether my partner will be taking part or not. and I never received any copy of the contract or receipt, therefore I don't have a account or policy number. Then later we decided we are not taking the contract and told her not to process it. The end of the month that month I received a debit sms and straight after the Sms I went to the branch and tried to terminate it only to find that that person no longer works there I spoke to a gentleman named Chilli about this and offered to pay the amount of cancellation fee but nothing has been done about it, and money has been deducted from my account all these months and now I don't live in Durban permanently. I have never been to the gym to actually gym there because I didn't want it to be processed to begin with.
