Debit order gone off without my consent

I took out a gym contract with Michael Rathbone on 27 April 2013. The terms and contract price were discussed and I paid for the contract in full on the day, Michael agreed that all amounts were paid in full and I could start gyming. At the end of May my bank account was debited with R3850 by Planet Fitness. I went into the bank and had this amount reversed. I phoned Planet Fitness to find out what was going on and they told me I still owed that amount on my contract. I asked that someone follow up with Michael, but no one ever did. At the end of June my bank account was debited again with R3850, I've tried to contact Michael several times but he won't take my calls. I got hold of Ravi, and Samantha who are in the process of helping me, however I haven't heard from either of them and the money has not been refunded to me. I am very upset about the matter. This was an error Michael made while filling out the contract with me and needs to be sorted out, the contract price was clearly stipulated on the day and it did not include this additional charge. Someone needs to contact me immediately to get this resolved.
