How many times do u need 2 request account closure

I paid my account in full as requested it to be closed assuming it was done I had them cut up the card and went on with my life several months later I was sent a lawyers letter saying my account was handed over I again paid the amount and requested the account to be closed. Last week again I got a letter showing that I had paid the money but again account still had credit. How many times does a person have to request an account to be closed If this matter is not settles asap I will be taking legal action.
