All I want is a new card

I have been in contact with the the customer care via email.
All I am requesting is a new club card reflecting my married name as well as to change my address!! I first requested this via email on September 23rd. I followed up again and again and was advised on 10/12 that the "issuse has been resolved"!!! NO IT HASN'T! I STILL don't have my new card.
I am using my old card still (not knowing if it still works), but have not received any vouchers or magazines from Clicks since JUNE 2013!!!
I find it unacceptable that I have had to wait and I am told over and over again that this is on the way!
At this point I am thinking it would have just been easier to open a whole new account with Clicks, but all my perscriptions from my doctors are the only thing stopping me!!!

Totally disjusted!
