Shocking Customer Service_Store Card

I have now applied twice to have my wife added onto my account, with a separate card.
The 1st time, in 2011, they lost all my documents; ID copies etc. Now in April 2013, I submitted another request at the Boskruin branch. I took copies of both ID copies in. I refused to have them taken and insisted they fax the application form and ID while I was there. This was submitted. I have now been waiting over 3 months for any feedback, card issue... I travel a bit but my wife isn't able to use it.
Please can someone let me know what the is going on...???
