Waiting 18 months for Unabbridged birth certificat

I have applied for an unabbridged birth certificate 18 months ago for my daugther at the Polokwane office during September 2011 and havent received it yet. I have been to the offices numerous times, but are being sent from one place to the next. I even completed a new application and attached a letter from the school that indicates the urgencey of the document, as my daugter needs it for a visa for a school trip.
In January, I was given the number of the call centre where I was given a case number and it would have been fast forwarded. I am still holding on, phoning every week and all I get is a promise and a request to phone again in two weeks.
I do not know what to do anymore as the deadline is end of May. Why is this department so incredibly slow or inefficient?
