Lost my phone, where's my claim form

I lost my phone which I purchased by fonechoice, I called the callcentre 2weeks ago
The consultant said I will receive a claim form to my email address I have not received Anything I called again they said they will send it on monday... I'm STILL waiting how am I supposed to claim? I need a phone and I have insurance
