Zero response after trying to add dependant

Spoke to Kim earlier about my application for adding an additional dependant.

The documents were submitted 30 Nov 2012, and had no response from Discovery. Apparently there was a request for me sent in December to provide missing info on the documents, which I never received.

Now I've been told that the application was cancelled without prior warning, and that I had to submit a new set of documents due to Discovery not accepting 60/90 day applications?

Why the do you make it my issue, if your company messed up with zero communications? No way in am I going to fill out that damned form again.
