Poor single mom cannot reduce premium

I have a few complaints:
1. I applied for my membership amount to be reduced as when i joined Medihelp my ex-husband was paying R9700 towards my son. He is now, if I am lucky pays R2000 or R2500 per month. This must pay for school fees (R1900), Medication Concerta (R553), Rent, food, clothing, stationery, and many more expenses. I am expected to pay all of these expenses from R2000 per month. I earn R8500 as a gross, not including overtime as some months there is no overtime. I asked for them to please help me and reduce the premium as my circumstances have changed... they have refused. Please can Medihelp tell me how to come out on R2000 per month and still afford the increase in premium and current premium?
2. I have supplied a claim for a physio Ms K. Laubscher for three consultations. 2 have been paid and the 3rd rejected due to no referral. 1st thing when I phoned Medihelp they said did not need a referral from GP, BUT I got one anyway. Why are they refusing to pay?
3. A testosterone test for M. Straw rejected as code not on form... On the 2014 doctors guide and 2013 lancet form it stands there.. but rejected, Why? I put in 2 motivations!!!
