2 x fold irritation at Discovery Health:

I had requested a Chronic application form from Discovery Health on 9 October 2013. I had not received the form so i called a week ago to request a form. To date none received. How do i obtain the form? do I need to drive to discovery offices, thought technology made our lives a smoother journey :-(((

I have resigned from my present company end October 2013. My medical aid savings have depleted in June 2013. I receive a debt statement from discovery health, stating i owe them monie! how is that so? can someone please explain as I pay cash presently when i do go to the doctors.

amazing how monies are taken so easily but service is below par :-(((
