Stinking attitude from Receptionist

Today at about 10:AM I called into the Dr's room to try and find out if my insurance form had been filled in, I spoke to the receptionist who is the same lady I left my forms with. Its been 2 weeks now going for the 3rd and till date I do not have forms that needed to be Filled in by the Dr. I expressed my concern regarding the fact that insurance have given me a cut off date to submit to them my Hospitalisation forms. 1stly I am required to pay them R500 just to fill out the form and as I was explaining that since I will be paying why am I not given them back to date? She just said if you want to ask me such things do it at your time I have other calls waiting and just hung-up on me. Atrocious attitude for someone who is suppose to be the face of the Dr what kind of reputation is she giving to us as patients?

Your attitude stinks and the Dr needs to take you to more Client Service Training.
