UNPAID PMB RELATED PATHOLOGY CLAIM

This is a complaint regarding MediHelp requesting a motivational letter for routine PMB pathology which have an auth number: 2007082200052 authorization number has remained the same since 2007 and authorization number is valid for the entire year for 4 x pathology tests, and there claim shows that there is a copayment, how can I have a copayment for PMB related services that are approved, bloods tariff codes approved are as follows: 3816/4429/4131/4001/4032/4071/3755 in conjuction with:3785/3791/3739/3762/3783-FBC.
My concern is that I have never being requested to have these services motivated for, as on my PMB careplan or basket of care, my physician, and general practitionerare approved for follow up consultations and can both can refer me for follow up bloods, suddenly after 6 years with same PMB authorization number, I am requested to forward a letter of motivation to have routine bloods done, (4 x consultations per annum).
