INCORRECT MONTHLY ON MY ACCOUNT

I have written to the Medical Aid on numerous occasions advising them that 2 items which appear on my monthly statement cannot reflect on my account namely Diabetes Mellitus Type 2 and Hypertension as I have never been diagnosed with either. I informed them each time that I suspect that these 2 illness related to my late husband and should be removed from my account. In the event of my wanting to take any sort of insurance, these 2 items are reflected as being a diagnosis made upon me. I want these 2 items removed from my monthly statement as the information is incorrect. Each time I have written, I have had a reply email that the statement will be amended and should reflect the following month and every month the statement arrives and the "offending" 2 items are on the list once again as a chronic diagnosis made on me. Please do I need to be reminded and be upset every month about my late husband. Can they not correct my statement once and for all
