Handed me to ITC, but never received statement

I went to the doctor in October 2012. I am on medical aid, but the money was paid into my bank account. Since January 2013 I struggle to get a statement from Intercare in Hazeldean. I phoned them numerous times to supply them with my email address as well as my fax number. Up until today i never received a statement and to my amusement on the 19 June 2013 I receive a summons from Schulenburg Attorneys requesting me to pay R755.00. Upon phone Schulenburg Attorneys I have spoken to a Mrs van Vuuren. She was very rude and every time I said to her i did not receive a statement and this was the first statement issued to me, the phone was dropped. I never said i will not pay the account. I only wanted a written statement as proof for my rekords to pay them.

This morning i receive a sms state that they have listed me as a bad payer on the credit bureau. If this is how they work with me, how do they work with their pasients.

I never received a statement through the post or even all the other relevant options that I gave them to communicate with me.
