Take my money refuses to give it back

This Guy is piece of work. Firstly he promises to come on certain date and then calls to say his wife had a miscarriage. The next time he promises to come and then calls in because his guys got know ID's and one guy is waiting for his ID from his mother in Kimberley etc. This guy is a piece of work who needs to be closed down and exposed. I am have now opened up a case against him. I cancelled this order and ask for my money back and he says i need to fill in vendor form to cancel the order. i am still waiting for the form and my money. STAT AWAY FROM THIS COMPANY
