What has happened to my pension payout

In February 2013 I resigned from my company called Mutual and Federal and completed all the necessary documentation in order for me to be retracted from the Old Mutual Pension Fund. Last week Friday, when we called Old Mutual, we were sent from Pillar to Post, just to be told that they have not received my original claim form. We then contacted my previous employers and they confirmed that they have sent everything to Old Mutual. This again seems like a case were Old Mutual is definitely playing with our future and delaying payout to someone! Old Mutual, this is my retirement money and we need this payout urgently. We have all the contact numbers for the executive at Old Mutual and will not hesitate to get in contact with them. Remember we worked for the sister company and will not hesitate to get in contact and burn the necessary fingers if needed.
