Loan "APPROVED" 3 times and then suddenly declined

On Monday 25 March 2013 I did an Internet application and were approved online provisionaly. A lady from African Bank Call Centre contacted me and did an assessment over the phone I was pre-approved for the second time for a loan and a credit card. I've asked her to give me a reduced amount because i don't need as much as i have been offered. On Saturday 30 March 2013 I went to my nearest branch and the loan and credit card for a third time was approved and I signed my terms and conditions and contract with debit order and everything and got my credit card which I also signed for and activated in the African Bank branch. The last thing that needed to be verified was my employment that would have not been a problem. Today I received a call from the branch to say "Your loan & card has been declined". I got approval and signed all documents, I do have copies of the contracts and still have the credit card. After numerous phone calls and agents, no one could explain to me why and said i must just throw the card away. Please can someone assist.
