Debiting Cheque Account without Authority

My Cheque account was debited at the end of Aug 2013 for an Clientele insurance policy. I have never been a client of Clientele. Now nearly two months down the line and numerous e -mails I have no feedback and also never received the debited amount back.

What must one DO!!!
