Broken cellphone

I have cellphone contract with BAYPORT CELLULAR. The phone just had a blank black screen and I reported it and they said I can take it to the nearest CELL C shop. Cell c phoned me back to tell me the phone is not theirs.
I collected the phone and phoned BAYPORT about the situation. They told me they will send a form for me to fill it and send it back to them of which I did several times cos every time I send it back I have filled it wrongly.
To this day after every phone call I made and filled in the forms several times, the phone has not being collected. I`m without a phone for 3 weeks now and no call from Bayport.
