Who is responsible??

Every month before even my salary reports on the phone, i have debit orders that i dont know of. If its not Dial-24, Reload, Nguni2 etc. going to the bank to reverse the debit does not help because still you get charged for those debits and come next month, the same happens. i went to the branch today and, nothing can be done to stop these debits.
