Repairs taking more than 3 months to be fixed

I took out a contract with Faranah last year for an iPhone 4.I told her to please activate the insurance. She agreed and ensured all will be done. I then took the phone into the store to be repaired on 13 October 2013, I was told it would take 7days. I went back to get the phone 10days later only to find out that Faranah did not activate the insurance. Ameer then said not to worry he will fix the error and have the phone repaired. It's been over 3months, the account is paid in cash every month and I still don't have a phone.when I call n ask I get told we will follow up n noone returns my call. Today when I called the store as I told Ameer last Monday that I want my phone by Friday 31January I find out that Ameer no longer works there and Faranah now gives me some story that because the first debit order did not go through hence the insurance was not activated and now she wants an affidavit in order to assist me. If that was the reason why did she not tell me this initially instead she tells me 3moths later. Faranah always has attitude and is rude and never gives you a chance to speak. I have been paying for a phone I don't have for over 3 months.
