Deduction of money from cancelled acct

Autopage deducting monthly amounts after I have cancelled the contract several times on completion of my 24 month contract.

This is the sequence of events:

I spoke to Catherine on 011-6502343 in May 2013 to cancel the contract.
In June I spoke to Shaun Moodley, Reference Number 2160130, he assured me my numbers have gone over to prepaid and that my contract has been cancelled
Then I spoke to Sophie Phiri on the 2 July 2013 in the accounts department on 011-6502740 to cancel the debit order. She assured me that it would be cancelled and no further debits would be taken off.
Once again I was debited on the 31st July 2013
I called again on the 1st August 2013 and spoke to May Mankoe where no record was found of my debit order going off and was transferred to the accounts department
On the 1st August 2013 I spoke to Marcia Mtshikukulume and was told to send my bankstatement to finance to investigate which I emailed to her on the same day.
2 September 2013 I have been debited again. No-one has bothered to phone me and I did not receive any replies to my emails.
