Refusing to pay my connection fee back to me

Nxatel deducted R620.00 from my bank account for their connection fee, this was more than a month ago, still no installation or connection has been made on their part.
I have cancelled my contract with them and filled in a refund form to claim my connection fee back and have sent it to them 4 times by email and have faxed it to them a further 3 times, they are ignoring my emails and phone calls, all I get is "they will come back to me in the next hour" but they never do.
The worst service I have ever encountered.
