Contracts on false names

In Feb 2009 I visited the gym to see if I like the place or not, they tricked me and said that they need my ID to controll who comes in and for a chance of vandalism. Now in Sept 2013 I founded that they opened an account on my name, flasified signatures and details, and started deducting money on my bank account, - R500 a mounth. I opend a case against them and they just keep going on and does not want to do anything about it. Every time I speak to someone, the next time either the person is not in, or the person left the branch.
I went to the bank to put a stop order on the amounts from them, but they keep changing the refrance number, in other words, the bank can't do anything about it.
Now apperiantly the gym's head office can't do anything about it and they say I need to contact an other company handling these cases, this company doesn't exist for all that matters.

Now the persons name mentioned as a consultant never worked at the company before so they can't trace this to any one.
