Debiting my account without my consent!!

I would like to log a complaint with regards to Jet stores. For several months now I realised on my statements that I am being charged for some hospital plan. Someone called me offering me the plan and I told that person that I am not interested. And according to my statement I have realised that the hospital plan is not the only insurance they are charging me for and last time I checked the only insurance that I am aware of is my cell phone insurance. I have send them faxes several times asking them to please cancel this insurances but till today it has not been done. I want them to cancel this insurances that they are charging me for and would like it to be done immediately. They also need to refund me for all these months that they have been charging me.
