My account having be Fica'd and it not being done

I had to Fica my Jet account and went to do so on the 23 October. When the slip kept on printing out so remind me to do it i thought i should check on it so went in in December only to be told oh well they have lost my form and proof of residence, by someone who clearly did not have a clue! i then get a phone call to say oh they found my stuff but the proof of residence has expired so back up to the Pavillion i go only to be confronted with another person with out a clue so i ask to speak to the manager, he takes the proof of residence and promises to fax it off. Clearly this never happened cause every time i use my card a slip prints to remind me to Fica my account. I have also phoned and emailed the relevent people and have never recieved a reply. Today i go to use my card only to find my account frozen!!! I did what i was suppose to do Jet has not done there job and i have been the one running around thanx to them and i am just really tired of there bad service my account is always payed every month and more is payed than is required!
