Account supposed to be close!!!

I went in November to close my late brothers account dealt with Amelda. She said everything is in order account is closed. Then in January they decided to call my brothers work as his account was never closed and his account his going in arrears went in again submitted all the documents was assured its closed. Now they sending letters that they going to black list him... My goodness how difficult can it be to close an account and what's the use in have cover when it can't cover whatever outstanding amount he had left no respect honestly!!!
