Arrangement made - ignored - charged double

I made a payment arrangement with Annelien - which was accepted and fine - yet three debit orders went off my account! One for full amount - another two for arranged amount which was supposed to go off! What is the use of dealing with the accounts department if they just do what they feel like doing?? Completely unacceptable especially when the fault is on their side cos they never changed banking details after 2 years of trying to change it!
