ID that was applied on 28 January 2013

My mother applied for a replacement of ID where a new surname was changed on the 28 January 2013; ID is not available for collection Home Affairs and according to them the surname has changed. No one at Home Affairs could give the reason as to why the ID is not available. The application was done at Home Affairs; Pretoria branch and prior to this few applications were done at Joburg branch; still no ID was issued. The date at Joburg branch refers 22 August 2007. This is an unnecessary stress to the family because it has an impact to life cover polices and other matters relating to the ID itself. We would like to receive a detailed information as to why the new ID with the correct surname (Makhanye) cannot be issued? The names in old ID should be Makhuzo Lettie Mhlambi; whereanew ID should reflect Makhuzo Lettie Makhanye.
