BI1663 form mixed up with another late person, as Department of Home Affairs picked the mix up. had to be reissued by Doctor, Hospital and Funeral services IN ORDER FOR DEATH CERTIFICATE TO BE ISSUED.
We then requested to identify the deceased as we where then not convinced that she was in Doves Funeral Services Mortuary. Even then we could not clearly identified the body as Funeral services Building property in darkness, there is/was electricity but no light bulbs.
