Billing on Medical Aid to be explained

I go to the medical center every second month for my contraceptives/family planning and i normally pay a cash fee of R50, as of this year a consultation fee of R120 is being charged and it has been billed on my medical aid (which i have full cover for the contraceptives as well) I have not been consulted and I've been billed the R120 on my medical aid and i had to pay the R50 cash as well. When i question the rip off, answers weren't given.what do i do in a case like this cause the medical is not free i do pay for it.
I want answers as to how the billing takes place and the consultation cause i haven't been consulted
